Overview

RH genotype-matched red blood cells are a specialized biologic intervention being evaluated for the treatment of pediatric patients with sickle cell disease (SCD) who require chronic blood transfusions. Developed by the Children's Hospital of Philadelphia in collaboration with the New York Blood Center, this approach involves transfusing red blood cells that are genetically matched to the recipient at the RHD and RHCE loci, rather than relying solely on standard serological matching. The donor units are typically sourced from Black and Hispanic populations to increase the likelihood of a genetic match. The primary goal is to prevent Rh alloimmunization, a common and serious complication in SCD where patients develop antibodies against foreign Rh antigens, which can lead to delayed hemolytic transfusion reactions and difficulty finding compatible blood in the future.
