Rhazya stricta alkaloidal extract, also known as Crude Alkaloid Extract of Rhazya stricta (CAERS), is a natural plant-derived extract obtained from the leaves of *Rhazya stricta* (harmal), a medicinal plant of the Apocynaceae family native to South Asia and the Middle East. The extract contains a complex mixture of indole alkaloids, glycosides, flavonoids, tannins, and triterpenes. Preclinically, CAERS has been investigated for its anticancer, antioxidant, antimicrobial, antidiabetic, and antihypertensive properties. In oncology research, it has demonstrated the ability to inhibit cell growth and induce apoptosis in various cancer cell lines, including non-small cell lung cancer (NSCLC), pancreatic cancer, breast cancer, glioblastoma, and colorectal cancer. Its apoptotic mechanism involves the regulation of the Bax/Bcl-2 ratio, the release of mitochondrial cytochrome c, and the activation of caspases 3 and 9. Additionally, CAERS has been shown to sensitize cancer cells to cisplatin chemotherapy. It remains a preclinical research compound with no commercial developers or approved clinical indications.
