**Rhenium-188-AHDD-Lipiodol** is a therapeutic radiopharmaceutical used for hepatic arterial radioembolization in hepatocellular carcinoma, particularly unresectable or advanced disease. It consists of the beta-emitting radionuclide rhenium-188 chelated by diacetylated hexadecyl diazathiol, a stabilized HDD-derived lipophilic chelator, and dispersed in Lipiodol. Following intra-arterial administration to the liver, Lipiodol preferentially accumulates in hypervascular hepatocellular carcinoma nodules, retaining the rhenium-188 complex and delivering localized beta radiation that produces tumor-cell DNA damage while limiting exposure of uninvolved liver tissue. The agent has been evaluated in clinical radioembolization and dosimetry studies and is reported to be available in India as REK-1.
