Rhenium-188 HDD iodized oil is a radiopharmaceutical composed of the beta-emitting radioisotope rhenium-188 complexed with the chelator HDD (4-hexadecyl 1-2,9,9-tetramethyl-4,7-diaza-1,10-decanethiol) and suspended in iodized oil (Lipiodol). It is primarily used via transarterial administration for targeted internal radiotherapy, particularly in the treatment of inoperable primary hepatocellular carcinoma (HCC). The mechanism of action involves embolization and selective delivery of high-energy beta radiation to hepatic tumors via the arterial supply, causing tumoricidal effects while minimizing radiation to normal tissue. Rhenium-188 provides both therapeutic beta emission for tumor destruction and gamma emission for imaging/dosimetry. The agent has been studied in multiple early phase clinical trials and multicenter studies, demonstrating a favorable safety profile and promising efficacy for HCC[1][2].
