Rhenium-188 lipiodol is a radiopharmaceutical agent used for the selective internal radiation therapy (SIRT) of inoperable hepatocellular carcinoma (HCC). It consists of the beta-emitting radioisotope rhenium-188 (188Re) complexed with a lipophilic chelating agent (such as HDD or DEDC) and dissolved in Lipiodol (ethiodized oil). Rhenium-188 is a high-energy beta emitter with a half-life of 16.9 hours, which also emits low-energy gamma rays suitable for imaging. Lipiodol acts as a carrier that is selectively retained in the neovasculature of liver tumors. When injected intra-arterially, the drug delivers targeted radiation directly to the tumor site while sparing healthy liver tissue. It is particularly valued in developing countries as a cost-effective alternative to yttrium-90 or iodine-131 therapies because rhenium-188 can be produced on-site using a tungsten-188/rhenium-188 generator.
