Overview

Riboflavin + dextran is a combination ophthalmic solution used as a photoenhancer in corneal collagen cross-linking (CXL) procedures. The formulation typically contains riboflavin 5’-phosphate (vitamin B2 derivative) at a concentration of approximately 0.1–0.146% and 20% dextran T-500 as an excipient to increase viscosity and maintain corneal structure during the procedure[2][4][8]. Riboflavin acts as a photosensitizer that, when activated by UVA light (370 nm), induces the formation of covalent bonds between collagen fibers in the cornea, thereby increasing its biomechanical strength and halting progression of ectatic disorders such as keratoconus and post-refractive surgery ectasia[4][8]. Dextran serves to control osmolarity and viscosity, ensuring even distribution of riboflavin within the stroma[7]. The most widely used commercial product is Photrexa Viscous. This drug is administered topically during CXL with the KXL System after epithelial debridement[3][4].
