Overview

The RIC Haplo-BM Transplant Protocol is a specialized hematopoietic stem cell transplantation (HSCT) regimen developed at the Masonic Cancer Center, University of Minnesota. This protocol utilizes a reduced-intensity conditioning (RIC) approach designed for patients with advanced or high-risk hematologic malignancies who lack a fully HLA-matched donor. The conditioning regimen consists of fludarabine, cyclophosphamide, and low-dose total body irradiation (TBI) to achieve lymphodepletion and immunosuppression. This is followed by the infusion of non-T-cell depleted bone marrow from an HLA-haploidentical (half-matched) related donor. To mitigate the risk of graft-versus-host disease (GVHD) inherent in haploidentical transplants, high-dose cyclophosphamide is administered post-transplant. This strategy significantly expands the donor pool, as nearly all patients have at least one haploidentical relative, while maintaining a potent graft-versus-leukemia effect.
