Overview

Ridinilazole is a novel, narrow-spectrum small molecule antibiotic developed for the treatment of *Clostridioides difficile* infection (CDI). It is administered orally and exhibits potent bactericidal activity specifically against *C. difficile*, with minimal impact on normal gut microbiota due to its high selectivity and poor systemic absorption. The primary mechanism of action involves binding to the DNA minor groove at AATTT-rich sequences in *C. difficile*, disrupting cell division and energy generation pathways, which leads to suppression of bacterial toxin production and reduced inflammation. Ridinilazole preserves key components of the human gut microbiome compared to standard therapies like vancomycin, potentially resulting in lower recurrence rates for CDI. The drug has been granted Qualified Infectious Disease Product (QIDP) status and Fast Track designation by the FDA[1][3][4][5][6][7].
