This is a combination therapy consisting of three small molecule drugs—rifabutin, lopinavir, and ritonavir—used primarily in the management of patients co-infected with HIV and tuberculosis. Rifabutin is an antibiotic from the rifamycin class that inhibits bacterial DNA-dependent RNA polymerase, thereby suppressing Mycobacterium tuberculosis replication. Lopinavir and ritonavir are antiretroviral protease inhibitors; ritonavir also acts as a pharmacokinetic booster by inhibiting cytochrome P450 3A (CYP3A), increasing plasma concentrations of lopinavir. This combination is used when standard rifampicin-based regimens are contraindicated due to drug-drug interactions with protease inhibitors in HIV therapy. The regimen requires careful dose adjustment due to significant pharmacokinetic interactions between these agents[1][3][6].
