Fixcom 2 is a fixed-dose combination of the first-line antituberculosis agents rifampicin (150 mg) and isoniazid (75 mg). Distributed by Natrapharm in the Philippines, it is indicated for the treatment of both pulmonary and extrapulmonary tuberculosis, particularly during the continuation phase of short-course treatment regimens. Rifampicin is a rifamycin antibiotic that inhibits bacterial DNA-directed RNA polymerase, thereby blocking RNA transcription. Isoniazid is a prodrug activated by the bacterial catalase-peroxidase enzyme KatG; its active form inhibits enoyl-[acyl-carrier-protein] reductase (InhA), an essential enzyme in mycolic acid biosynthesis required for bacterial cell wall assembly. Together, these agents exert synergistic bactericidal and sterilizing activity against Mycobacterium tuberculosis, simplifying dosing regimens and improving patient compliance to prevent the emergence of drug resistance.
