Overview

9RLE is a specialized anti-tubercular therapeutic regimen composed of rifampicin, levofloxacin, and ethambutol, intended for a treatment duration of nine months. This regimen was investigated by the Institute of Liver and Biliary Sciences (ILBS) in India specifically for the treatment of tuberculosis in patients with concurrent chronic liver disease (CLD), including both compensated and decompensated states. By excluding isoniazid and pyrazinamide—standard first-line agents with high hepatotoxic potential—and incorporating the fluoroquinolone levofloxacin, the 9RLE regimen aims to provide an effective treatment for *Mycobacterium tuberculosis* while reducing the risk of drug-induced liver injury in a vulnerable population. Rifampicin acts by inhibiting bacterial DNA-dependent RNA polymerase, levofloxacin targets DNA gyrase and topoisomerase IV, and ethambutol disrupts cell wall synthesis by inhibiting arabinosyl transferase.
