Overview

Rikkunshito (TJ-43) is a standardized Japanese Kampo medicine composed of eight herbal extracts: Atractylodis Lanceae Rhizoma, Panacis Rhizoma, Pinelliae Tuber, Hoelen, Aurantii Nobilis Pericarpium, Zizyphi Fructus, Glycyrrhizae Rhizoma, and Zingiberis Rhizoma. It is primarily used for the treatment of functional dyspepsia (FD), gastroesophageal reflux disease (GERD), and anorexia. The drug's mechanism of action is centered on the ghrelin system; it stimulates the secretion of ghrelin from the stomach and enhances the signaling of the growth hormone secretagogue receptor (GHS-R1a). Additionally, it acts as an antagonist at 5-HT2B and 5-HT2C receptors, which helps to counteract the suppression of ghrelin release. These actions collectively improve gastric emptying, enhance gastric accommodation, and stimulate appetite. Rikkunshito is manufactured by Tsumura & Co. and is widely prescribed in Japan.
