Rilpivirine acts by inhibiting the HIV-1 reverse transcriptase enzyme, preventing viral replication[4][8]. Rifabutin inhibits bacterial DNA-dependent RNA polymerase, suppressing RNA synthesis and leading to cell death, and acts as a moderate inducer of the cytochrome P450 3A4 (CYP3A4) enzyme[2]. Their combination is clinically significant because rifabutin, as a CYP3A4 inducer, can reduce rilpivirine plasma concentrations potentially requiring rilpivirine dose adjustments if used together[5][1].
