Rimegepant is a small molecule calcitonin gene-related peptide (CGRP) receptor antagonist used for both the acute treatment of migraine with or without aura and the preventive treatment of episodic migraine in adults. It works by binding with high affinity to the human CGRP receptor, thereby blocking CGRP activity and preventing vasodilation and decreased excitability of Aδ nerves associated with migraine pathophysiology. Unlike triptans, it does not cause vasoconstriction, making it suitable for patients with cardiovascular contraindications to triptans. Rimegepant was originally developed by Biohaven Pharmaceuticals and is marketed under the brand names Nurtec ODT (United States) and Vydura (European Union)[1][3][5][7][8].
