Drug intelligence / Profile preview
DM199 (rinvecalinase alfa) is a recombinant form of human tissue kallikrein-1 (rhKLK1) being developed by DiaMedica Therapeutics for the treatment of acute ischemic stroke (AIS) and preeclampsia. KLK1 is a serine protease that plays a crucial role in physiological regulation by increasing the production of nitric oxide, prostacyclin, and endothelium-derived hyperpolarizing factors. In the context of AIS, DM199 acts as a bradykinin-producing enzyme that aims to improve clinical outcomes by enhancing collateral circulation to the ischemic penumbra, stimulating angiogenesis, and promoting cellular repair mechanisms. It is currently being evaluated in the Phase 2/3 ReMEDy2 trial for patients with moderate stroke severity who present within 24 hours of symptom onset and are not candidates for mechanical thrombectomy.
