A combination therapy consisting of risperidone, an atypical antipsychotic, and desipramine, a tricyclic antidepressant. **Risperidone** works primarily by blocking dopamine D2 receptors and serotonin 5-HT2A receptors. It has a transient and loose affinity for D2 receptors, aiming for 60-70% occupancy, which helps reduce positive symptoms of schizophrenia. High-affinity binding to 5-HT2A receptors may address negative symptoms. **Desipramine** is a selective norepinephrine reuptake inhibitor (NRI), blocking the reuptake of norepinephrine (noradrenaline) from the neuronal synapse. It has less potent inhibition of serotonin reuptake. Chronic use can lead to down-regulation of beta-adrenergic receptors and sensitization of serotonergic receptors. This combination has been studied for its potential enhanced therapeutic effects, particularly in reducing alcohol consumption in experimental models, suggesting a potentially synergistic effect by addressing multiple neurotransmitter systems simultaneously. There is also potential application in treatment-resistant depression.
