This is a combination of two atypical antipsychotic drugs, risperidone and zotepine. Both agents are used primarily in the treatment of schizophrenia and related psychotic disorders. Risperidone acts mainly as an antagonist at dopamine D2 and serotonin 5-HT2A receptors, reducing positive symptoms (such as hallucinations and delusions) while also modulating negative symptoms through serotonergic pathways. Zotepine is also a dopamine antagonist with high affinity for D1- and D2-like receptors, but it additionally exhibits strong antagonism at several serotonin receptor subtypes (including 5-HT2A, 5-HT2C, 5-HT6, and 5-HT7), inhibits noradrenaline reuptake, and has sedative properties. The combination may be considered in clinical scenarios where monotherapy is insufficient or specific symptom profiles warrant dual pharmacological action; however, there is limited evidence supporting routine combined use over monotherapy with either agent[1][4][5]. Both drugs are small molecules.
