Drug intelligence / Profile preview
Risperidone LAI is a long-acting injectable formulation of the atypical antipsychotic risperidone. It comes in several branded versions including Risperdal Consta, Perseris, Uzedy, Rykindo, and Risvan. These formulations differ in their administration methods (intramuscular vs subcutaneous) and dosing intervals (biweekly vs monthly). ## Formulations and Administration Risperidone LAI is available in several extended-release injectable formulations: - **Risperdal Consta/Rykindo/Risvan**: Intramuscular injection administered every 2 weeks (Risperdal Consta, Rykindo) or monthly (Risvan) into the buttocks or upper arm. - **Perseris**: Subcutaneous injection administered monthly in the upper arm or abdomen, with no loading dose or oral supplementation needed. - **Uzedy**: Subcutaneous injection administered monthly or every 2 months in the upper arm or abdomen. When initiating treatment with some formulations (particularly Risperdal Consta), patients may need to continue oral risperidone for 1-3 weeks after the first injection. ## Clinical Use and Efficacy Risperidone LAI is primarily indicated for: - Treatment of schizophrenia in adults - Maintenance treatment of bipolar I disorder (Risperdal Consta) Clinical studies have shown that risperidone LAI provides: - Superior efficacy compared to oral antipsychotics when patients are switched to the injectable form - Significant reduction in total PANSS (Positive and Negative Syndrome Scale) scores - Extended protection against relapse, with projected median time to relapse of 161.8 weeks with 25 mg and 259.0 weeks with 50 mg doses ## Advantages and Patient Acceptance The long-acting formulations offer several benefits: - Improved treatment adherence through extended dosing intervals (1-2 months) - Quick achievement of therapeutic blood concentrations (within 6-24 hours for Uzedy, 4-6 hours for Perseris) - High patient satisfaction (92% of patients reported satisfaction with Uzedy) - Better injection experience compared to previous LAIs (70% of patients who previously used LAIs preferred Uzedy) ## Safety Considerations Risperidone LAI is not FDA-approved for treating behavior problems in older adults with dementia. Like other antipsychotics, it carries a boxed warning about increased mortality in elderly patients with dementia-related psychosis. The extended-release formulations have shown a low incidence of treatment-emergent extrapyramidal side effects and are generally well-accepted by patients.
