Overview

Ritonavir + dabigatran is a combination of two pharmaceutical agents used concomitantly, often in patients with comorbid conditions such as HIV (requiring ritonavir) and atrial fibrillation or thromboembolic disorders (requiring dabigatran). Ritonavir is an antiretroviral protease inhibitor primarily used as a pharmacokinetic enhancer due to its potent inhibition of cytochrome P450 3A4 and P-glycoprotein (P-gp), thereby increasing the plasma concentrations of coadministered drugs. Dabigatran is an oral direct thrombin inhibitor anticoagulant that prevents clot formation by inhibiting the conversion of fibrinogen to fibrin. When administered together, there is a significant potential for drug-drug interaction because ritonavir can inhibit P-gp, leading to increased exposure and anticoagulant effect of dabigatran, which may raise bleeding risk[1][3][5][8]. Dose adjustments or alternative therapies may be required depending on renal function and clinical context.
