RB + RBAC is a combination chemotherapy regimen consisting of rituximab and bendamustine (RB) alternating with rituximab, bendamustine, and cytarabine (RBAC). This alternating protocol is primarily investigated for the first-line treatment of elderly patients with newly diagnosed mantle cell lymphoma (MCL) who are ineligible for high-dose therapy and autologous stem cell transplantation. The regimen utilizes three active agents with distinct mechanisms: rituximab, a chimeric monoclonal antibody that targets the CD20 antigen on the surface of B-cells; bendamustine, a bifunctional alkylating agent that induces DNA cross-linking and strand breaks; and cytarabine, a nucleoside analog that interferes with DNA synthesis by inhibiting DNA polymerase and incorporating into DNA strands. The alternating schedule is designed to enhance efficacy and overcome resistance compared to standard rituximab-bendamustine therapy alone.
