Overview

This regimen is a sequential combination chemotherapy protocol used primarily for the treatment of aggressive B-cell non-Hodgkin lymphoma, including mantle cell lymphoma and relapsed/refractory cases. It consists of two established multi-agent regimens administered in sequence: - **Rituximab + CHOP** (R-CHOP): Combines rituximab (a monoclonal antibody targeting CD20 on B-cells) with cyclophosphamide, doxorubicin, vincristine, and prednisolone. R-CHOP is a standard first-line therapy for many B-cell lymphomas. - **Rituximab + DHAOx** (R-DHAOx): Combines rituximab with dexamethasone, high-dose cytarabine (ara-C), and oxaliplatin. This regimen is often used as salvage therapy or as part of induction prior to autologous stem cell transplantation. The sequential use of these regimens aims to maximize tumor cytoreduction and improve outcomes before consolidation with stem cell transplant or maintenance therapy. Rituximab acts via immune-mediated mechanisms against CD20-positive cells; CHOP components are classic cytotoxic agents; DHAOx substitutes oxaliplatin for cisplatin to reduce nephrotoxicity while maintaining efficacy[1][2][4][6][7].
