Overview

R-CHOEP is an intensified chemoimmunotherapy regimen used primarily for the treatment of aggressive non-Hodgkin lymphomas, such as diffuse large B-cell lymphoma (DLBCL), particularly in younger, high-risk patients. It consists of the monoclonal antibody rituximab (targeting CD20) combined with the CHOEP chemotherapy backbone: cyclophosphamide (an alkylating agent), doxorubicin (an anthracycline and topoisomerase II inhibitor), vincristine (a microtubule inhibitor), etoposide (a topoisomerase II inhibitor), and prednisone (a corticosteroid). The addition of etoposide to the standard R-CHOP regimen is intended to improve outcomes in younger patients with high-risk features, although it is associated with increased hematologic toxicity. It is often administered in dose-dense schedules, such as R-CHOEP-14, which utilizes growth factor support to maintain a 14-day cycle interval.
