R-CHOP + R-DHAOx + zanubrutinib is a combination chemoimmunotherapy regimen being investigated for the treatment of young, fit patients with previously untreated mantle cell lymphoma (MCL). The regimen alternates three cycles of R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) with three cycles of R-DHAOx (rituximab, dexamethasone, high-dose cytarabine, and oxaliplatin), with the addition of the next-generation Bruton's tyrosine kinase (BTK) inhibitor zanubrutinib during induction cycles. This approach, evaluated in the Phase II BRIDGE study (NCT04736914) sponsored by Sun Yat-sen University Cancer Center, aims to deepen clinical responses and potentially reduce the toxicity associated with intensive cytarabine-based chemotherapy by incorporating highly selective BTK inhibition. Following induction, patients may proceed to autologous stem cell transplantation (ASCT) and receive zanubrutinib maintenance therapy.
