Overview

This treatment plan is a chemo-immunotherapy and radiotherapy regimen developed by the Fondazione Italiana Linfomi (FIL) for the treatment of diffuse large B-cell lymphoma (DLBCL). The protocol involves six cycles of R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) administered in either 14-day or 21-day intervals. Following chemotherapy, patients undergo PET-scan restaging; those who remain PET-positive receive consolidation radiotherapy (36 Gy) to the site of initial bulky disease or residual PET-avid areas. This risk-adapted approach aims to maximize cure rates in low-risk DLBCL patients (aaIPI 0 with bulky disease or aaIPI 1) while using PET imaging to guide the necessity of local radiation therapy.
