This investigational combination therapy, developed by The University of Texas Health Science Center at San Antonio, integrates the standard-of-care R-CHOP chemoimmunotherapy regimen with roflumilast, an oral phosphodiesterase 4 (PDE4) inhibitor. R-CHOP consists of rituximab (a CD20-targeting monoclonal antibody), cyclophosphamide (an alkylating agent), doxorubicin hydrochloride (an anthracycline), vincristine sulfate (a vinca alkaloid), and prednisone (a corticosteroid). The addition of roflumilast is intended to enhance therapeutic efficacy in high-risk diffuse large B-cell lymphoma (DLBCL) by inhibiting PDE4, which suppresses B-cell receptor signaling, PI3K/AKT activity, and angiogenesis within the lymphoma microenvironment. This regimen has been evaluated in Phase I and Phase II clinical trials for treatment-naïve patients with moderate-to-high risk DLBCL.
