R-CVP is a chemoimmunotherapy combination regimen comprising the anti-CD20 monoclonal antibody rituximab and three chemotherapy agents: cyclophosphamide (an alkylating agent), vincristine (a vinca alkaloid microtubule inhibitor), and prednisone (a corticosteroid). This regimen is frequently employed in the treatment of various B-cell non-Hodgkin lymphomas, including follicular lymphoma and marginal zone lymphoma. In the context of diffuse large B-cell lymphoma (DLBCL), R-CVP is often investigated as a less toxic alternative to the standard R-CHOP regimen for elderly or frail patients who cannot tolerate anthracyclines like doxorubicin or epirubicin. The regimen works through a multi-targeted approach: rituximab induces B-cell lysis via complement-dependent cytotoxicity and antibody-dependent cellular cytotoxicity, cyclophosphamide causes DNA cross-linking to inhibit replication, vincristine disrupts mitotic spindles to arrest cell division, and prednisone exerts lympholytic effects through glucocorticoid receptor activation.
