REPOCH (also known as R-EPOCH) is an intensified immunochemotherapy regimen used for the treatment of aggressive B-cell non-Hodgkin lymphomas, including high-grade diffuse large B-cell lymphoma (DLBCL) and "double-hit" or "triple-hit" lymphomas. The regimen consists of the anti-CD20 monoclonal antibody rituximab combined with the EPOCH chemotherapy backbone, which includes etoposide, prednisone, vincristine, cyclophosphamide, and doxorubicin. It is often administered as a dose-adjusted (DA) regimen, where chemotherapy doses are titrated based on patient-specific hematologic toxicity to optimize the dose-intensity. Clinical evidence, such as the CALGB 50303 trial and subsequent meta-analyses, has evaluated REPOCH against the standard R-CHOP regimen, showing potential benefits in high-risk subgroups despite increased toxicity.
