This is a combination immunosuppressive therapy regimen consisting of rituximab, a chimeric monoclonal antibody targeting the CD20 antigen on B-cells, and everolimus, a small molecule inhibitor of the mammalian target of rapamycin (mTOR). Investigated by National Taiwan University Hospital in a phase 4 clinical trial (NCT01312064), this regimen is used for induction and maintenance immunosuppression in renal transplant recipients. Rituximab facilitates B-cell depletion to prevent antibody-mediated rejection, while everolimus provides a calcineurin inhibitor (CNI)-free or CNI-reduced maintenance strategy to mitigate the nephrotoxic effects typically associated with standard tacrolimus-based therapies, thereby aiming to preserve renal function in patients with or at risk of renal insufficiency.
