Overview

Rituximab + gemcitabine + dexamethasone + cisplatin (commonly known as the R-GDP regimen) is a multi-agent chemotherapy protocol primarily used as salvage therapy for relapsed or refractory aggressive B-cell lymphomas such as diffuse large B-cell lymphoma (DLBCL). The regimen combines four agents with distinct mechanisms: - Rituximab is a monoclonal antibody targeting CD20 on B cells and mediates cell lysis via immune mechanisms. - Gemcitabine is a pyrimidine antimetabolite that inhibits DNA synthesis. - Dexamethasone is a synthetic glucocorticoid with anti-inflammatory and immunosuppressive effects. - Cisplatin is an alkylating agent that forms DNA crosslinks leading to apoptosis. The combination aims to maximize cytotoxicity while allowing outpatient administration and facilitating stem cell mobilization prior to autologous transplantation. The addition of rituximab enhances efficacy in CD20-positive lymphomas. This protocol has demonstrated effectiveness in phase III trials for relapsed/refractory aggressive lymphoma[1][2][6].
