Overview

This combination regimen consists of five drugs: rituximab, ifosfamide, carboplatin, etoposide, and bortezomib. Rituximab is a monoclonal antibody targeting CD20 on B cells, leading to cell lysis via immune-mediated mechanisms. Ifosfamide, carboplatin, and etoposide are chemotherapeutic agents with cytotoxic effects on rapidly dividing cells. Bortezomib is a proteasome inhibitor that disrupts proteasome-mediated degradation of pro-apoptotic factors, inducing apoptosis in cancer cells. This combination is under investigation primarily for relapsed or refractory aggressive B-cell lymphomas, including diffuse large B-cell lymphoma, and in particular settings such as HIV-associated lymphomas. The regimen seeks to leverage the synergistic lethal effects of combining an anti-CD20 monoclonal antibody, multiple cytotoxic chemotherapies, and a proteasome inhibitor to increase tumor cell kill and potentially overcome resistance mechanisms[4][6].
