This combination chemotherapy regimen, developed at Memorial Sloan Kettering Cancer Center (MSKCC), is a dose-escalated salvage therapy designed for patients with primary refractory or poor-risk relapsed aggressive B-cell non-Hodgkin lymphoma. The regimen, often referred to as R-ICEesc or augmented R-ICE, consists of the monoclonal antibody rituximab combined with the ICE chemotherapy backbone (ifosfamide, carboplatin, and etoposide/VP-16). Rituximab targets CD20-positive B-cells, while the chemotherapy components act as DNA alkylating agents and topoisomerase II inhibitors to induce tumor cell death. The regimen also includes mesna as a uroprotective agent to prevent ifosfamide-induced hemorrhagic cystitis and G-CSF (granulocyte colony-stimulating factor) to support hematopoietic recovery and facilitate stem cell mobilization. It is typically administered for two cycles as a bridge to autologous stem cell transplantation (ASCT).
