Modified R-MINE is a combination chemotherapy regimen consisting of rituximab, ifosfamide, mesna, mitoxantrone hydrochloride liposome (PLM60), and etoposide. Developed by The First Affiliated Hospital with Nanjing Medical University, this regimen is specifically designed for patients with late relapsed diffuse large B-cell lymphoma (DLBCL). The key innovation is the substitution of conventional mitoxantrone with a liposomal formulation (PLM60), which aims to enhance efficacy and reduce cardiotoxicity. Rituximab targets CD20-positive B-cells, while ifosfamide, mitoxantrone, and etoposide act as cytotoxic agents through DNA alkylation, intercalation, and topoisomerase II inhibition. Mesna is included to prevent hemorrhagic cystitis associated with ifosfamide use.
