Overview

The combination of rituximab and lenalidomide (often referred to as "R2") is an established chemotherapy-free regimen for the treatment of relapsed or refractory indolent non-Hodgkin lymphoma (iNHL), particularly follicular lymphoma (FL) and marginal zone lymphoma (MZL)[5][3][4]. Rituximab is a monoclonal antibody targeting CD20 on B cells, leading to their depletion through mechanisms such as antibody-dependent cellular cytotoxicity (ADCC) and complement-mediated lysis. Lenalidomide is an immunomodulatory small molecule that enhances immune cell function by altering cytokine production, regulating T cell co-stimulation, enhancing NK cell-mediated cytotoxicity, and modulating the cereblon E3 ubiquitin ligase complex to degrade key transcription factors in malignant B cells[6][1]. The combination acts synergistically by reactivating dysfunctional T and NK cells in FL patients and improving ADCC against tumor cells[1]. Clinical trials have shown higher response rates and longer progression-free survival with this regimen compared to rituximab monotherapy[8][3].
