Overview

This is a combination chemotherapy regimen consisting of rituximab, methotrexate (typically high-dose), and temozolomide. It is primarily used in the treatment of primary central nervous system lymphoma (PCNSL), a rare form of non-Hodgkin lymphoma. Rituximab is a monoclonal antibody targeting CD20 on B cells, leading to their depletion via immune-mediated mechanisms. Methotrexate is an antimetabolite and antifolate agent that inhibits dihydrofolate reductase, disrupting DNA synthesis and cell replication. Temozolomide is an oral alkylating agent that methylates DNA at the O6 and N7 positions of guanine, resulting in cytotoxicity especially in rapidly dividing tumor cells. The combination aims to improve response rates over single-agent or dual-agent regimens while maintaining manageable toxicity profiles[1][2][8]. This regimen has been studied as both induction and maintenance therapy for PCNSL.
