Overview

**Rituximab + methotrexate + thiotepa (R-MT)** is a combination chemotherapy regimen used as first-line induction therapy primarily for primary central nervous system lymphoma (PCNSL) and central nervous system diffuse large B-cell lymphoma (CNS-DLBCL). Rituximab is a chimeric monoclonal antibody that targets CD20 on B-cells, inducing antibody-dependent cellular cytotoxicity, complement-dependent cytotoxicity, and apoptosis. Methotrexate, an antimetabolite, inhibits dihydrofolate reductase to block DNA synthesis, particularly effective at high doses for CNS penetration. Thiotepa, an alkylating agent, crosslinks DNA guanine bases to prevent replication. This trio leverages CNS-penetrating properties for improved complete remission rates (up to 49% in trials) over methotrexate-cytarabine alone, though associated with higher hematological toxicity.[1][4][12]
