Rivaroxaban + aspirin

Rivaroxaban + aspirin is a fixed-dose combination therapy used to reduce the risk of major adverse cardiovascular events in patients with chronic coronary artery disease (CAD) or peripheral artery disease (PAD). Rivaroxaban is an oral, direct factor Xa inhibitor anticoagulant, while aspirin (acetylsalicylic acid) is an antiplatelet agent that irreversibly inhibits cyclooxygenase-1 (COX-1), reducing thromboxane A2 production and platelet aggregation. The combination has demonstrated superior efficacy compared to aspirin alone in reducing the incidence of stroke, myocardial infarction, acute limb ischemia, and cardiovascular mortality in high-risk patients. This benefit was shown in large clinical trials such as COMPASS and VOYAGER PAD[4][5][6][8]. The mechanism involves dual inhibition of thrombosis pathways: rivaroxaban targets coagulation via factor Xa inhibition, while aspirin inhibits platelet activation.
