Rivaroxaban + tranexamic acid is a combination therapeutic regimen consisting of the direct factor Xa inhibitor rivaroxaban and the antifibrinolytic agent tranexamic acid. This combination is primarily investigated for its efficacy in managing the competing risks of perioperative blood loss and postoperative venous thromboembolism (VTE) in patients undergoing major orthopedic surgeries, such as those for intertrochanteric fractures. Tranexamic acid works by competitively inhibiting the activation of plasminogen to plasmin, thereby stabilizing fibrin clots and reducing surgical bleeding. Rivaroxaban, an oral anticoagulant, selectively inhibits factor Xa, which interrupts the intrinsic and extrinsic pathways of the blood coagulation cascade, preventing thrombin formation and the development of thrombi. Research conducted at institutions like Qilu Hospital of Shandong University has focused on optimizing the timing and dosage of these two agents to improve clinical outcomes in elderly orthopedic trauma patients.
