Rivipansel is a synthetic glycomimetic small molecule that acts as a pan-selectin antagonist, primarily inhibiting E-selectin but also P-selectin and L-selectin. It was developed for the treatment of vaso-occlusive crisis (VOC) in sickle cell disease. By blocking selectins, which are cell adhesion molecules involved in leukocyte-endothelial interactions, rivipansel prevents the adhesion and activation of blood cells to the endothelium. This mechanism aims to reduce inflammation, normalize blood flow, and decrease pain crises associated with sickle cell disease. Rivipansel was developed by GlycoMimetics in partnership with Pfizer and reached phase III clinical trials; however, it did not meet its primary efficacy endpoints in these studies[1][3][5][7].
