RLT-173 is a novel, monodisperse PEGylated peptide therapeutic developed by Realta Life Sciences. The drug consists of a 15-amino acid peptide sequence (IALILEPICCQERAA) derived from the C-terminal region of alpha-1 antitrypsin (AAT), which is conjugated to a monodisperse polyethylene glycol (PEG) tail to optimize its pharmacokinetic profile. RLT-173 functions as a potent modulator of the innate immune system, specifically targeting and inhibiting the activation of the NLRP3 inflammasome. By preventing the assembly of the NLRP3 complex, it reduces the maturation and release of pro-inflammatory cytokines such as IL-1β and IL-18. This mechanism allows for the resolution of hyper-inflammation in acute conditions without inducing broad-spectrum immunosuppression. RLT-173 is primarily being developed for the treatment of steroid-refractory acute graft-versus-host disease (aGVHD) and has also been investigated for acute respiratory distress syndrome (ARDS).
