Overview

RM-048 is a potent and selective cyclic peptide agonist of the melanocortin-4 receptor (MC4R), developed by Rhythm Pharmaceuticals. It was designed to target the leptin-melanocortin pathway, a critical hypothalamic circuit involved in the regulation of appetite and energy homeostasis. RM-048 was specifically engineered to address the limitations of earlier MC4R agonists, which were often associated with adverse cardiovascular effects such as hypertension and tachycardia. Preclinical studies, including those in non-human primates, demonstrated that RM-048 could effectively reduce food intake and induce significant weight loss without causing the pressor effects seen with first-generation compounds. Although RM-048 showed promise as a therapeutic candidate for genetic forms of obesity and hyperphagia, Rhythm Pharmaceuticals ultimately prioritized the clinical development and regulatory approval of setmelanotide (RM-493), leaving RM-048 as a secondary or backup candidate in their portfolio.
