RMG-02 is a proprietary liquid formulation of the Class 1C antiarrhythmic agent **flecainide acetate**, currently under development by **RxOmeg Therapeutics** (also known as Romeg Therapeutics). It is specifically being developed for the treatment of **Catecholaminergic Polymorphic Ventricular Tachycardia (CPVT)**, a rare genetic cardiac condition characterized by exercise- or emotion-induced ventricular arrhythmias. Flecainide acts primarily by blocking the **Nav1.5** voltage-gated sodium channels, which slows cardiac conduction. Additionally, in the context of CPVT, flecainide is thought to directly inhibit the **Ryanodine Receptor 2 (RyR2)**, reducing the frequency of spontaneous calcium release from the sarcoplasmic reticulum that triggers delayed afterdepolarizations. The liquid formulation is intended to facilitate precise weight-based dosing and improve ease of administration for pediatric patients, who represent a significant portion of the CPVT population. RMG-02 has received **Orphan Drug Designation** from the FDA and is currently in **Phase 2** clinical development.
