Recombinant neuregulin 4 (rNRG4) is a laboratory-synthesized protein form of neuregulin 4 (Nrg4), an adipokine secreted primarily by adipose tissue. Nrg4 acts as a ligand for the ErbB4 tyrosine kinase receptor, modulating metabolic homeostasis by stimulating ErbB4 signaling in target tissues, including the hypothalamus and endothelial cells. rNRG4 has shown potential in reducing obesity and related metabolic disorders by acting centrally to influence energy expenditure, suppressing appetite through ErbB4-oxytocin neuron signaling in the hypothalamus, and regulating inflammation in vascular endothelium via inhibition of the NF-κB pathway. It is being investigated primarily for metabolic disease (obesity, metabolic syndrome), and preclinical studies indicate additional benefit in vascular inflammation and atherosclerosis.
