Ro 27-2771 is an investigational small molecule dual integrin antagonist developed by Hoffmann-La Roche for the treatment of persistent asthma. It functions by blocking the $\alpha_4\beta_1$ (also known as Very Late Antigen-4 or VLA-4) and $\alpha_4\beta_7$ integrins, which are critical for the adhesion and transendothelial migration of inflammatory cells, such as eosinophils and T-lymphocytes, into airway tissues. By inhibiting the interaction of these integrins with their respective ligands, vascular cell adhesion molecule-1 (VCAM-1) and mucosal addressin cell adhesion molecule-1 (MAdCAM-1), the drug aims to attenuate the chronic airway inflammation characteristic of asthma. Ro 27-2771 was primarily evaluated in clinical development as its oral prodrug form, Ro 27-2441. A Phase 2 dose-ranging study was conducted to assess its efficacy and safety in patients with persistent asthma who were not receiving inhaled corticosteroids, but the development program was subsequently discontinued.
