Overview

RO4597014 is a small molecule glucokinase activator (GKA) developed by Roche for the treatment of type 2 diabetes. It was identified as a clinical candidate optimized to resolve metabolite redox cycling issues associated with earlier compounds in the same class, such as piragliatin. Glucokinase serves as a glucose sensor in the pancreas and liver; its activation enhances glucose-stimulated insulin secretion from pancreatic beta cells and promotes glucose uptake and glycogen synthesis in the liver. Although RO4597014 demonstrated robust glucose-lowering effects in preclinical models and was evaluated in clinical trials, development of this specific candidate and many others in the GKA class was ultimately discontinued due to challenges common to the mechanism, including the risk of hypoglycemia and concerns regarding the durability of the glucose-lowering effect.
