Overview

RO7027535 (also known as RG7827 or CEA-4-1BBL) is a tumor-targeted immunotherapy developed by Roche. It is a recombinant fusion protein consisting of a trimeric human 4-1BB ligand (4-1BBL) linked to a humanized antibody fragment (Fab) specific for carcinoembryonic antigen (CEA). The molecule is designed to provide potent T-cell co-stimulation via the 4-1BB (CD137) receptor specifically within the tumor microenvironment, thereby minimizing the systemic toxicities often associated with non-targeted 4-1BB agonists. By binding to CEA-expressing tumor cells, RO7027535 clusters 4-1BB on infiltrating T cells, enhancing their effector function and survival. It is primarily investigated in combination with T-cell bispecific antibodies (such as cibisatamab) or checkpoint inhibitors for the treatment of CEA-positive solid tumors, including colorectal and gastric cancers.
