RO7119929 is an orally bioavailable small molecule prodrug that acts as a selective agonist of Toll-like receptor 7 (TLR7). Upon oral administration, it is rapidly converted in hepatocytes to its active form, RO7117418. The drug stimulates the innate immune system by activating TLR7, leading to the induction of cytokines and chemokines and reprogramming myeloid cells within the tumor microenvironment. This immunostimulatory effect promotes local inflammation and enhances antitumor immune responses, particularly through activation of CD8+ effector T cells. Developed by Hoffmann-La Roche, RO7119929 has been investigated primarily for unresectable advanced or metastatic primary liver cancers and other solid tumors with predominant liver involvement. As of 2023, it has completed Phase 1 clinical trials evaluating safety, pharmacodynamics, pharmacokinetics, and preliminary antitumor activity[1][2][3][4][5][6][7].
