Overview

RO7223619 (also known as RG6333) is a novel recombinant fusion protein developed by Roche that functions as a CD19-targeted 4-1BB (CD137) agonist. The molecule is engineered to provide localized costimulation to immune cells by clustering 4-1BB receptors upon binding to CD19-expressing cells. It typically consists of a CD19-binding moiety fused to the 4-1BB ligand (4-1BBL) and an effector-silent Fc region. While primarily investigated for B-cell hematological malignancies where CD19 is endogenously expressed, RO7223619 is also being explored for the treatment of solid tumors. In this context, it is used in combination with agents such as CD19-encoding oncolytic viruses (e.g., RG6169) that induce de novo CD19 expression on the surface of solid tumor cells, thereby creating a "dock" for the fusion protein to activate T-cells and natural killer (NK) cells specifically within the tumor microenvironment.
