RO7268489 (also known as RG6367) is an investigational small molecule inhibitor of monoacylglycerol lipase (MAGL) being developed by Roche for the treatment of progressive forms of multiple sclerosis (PMS). MAGL is the primary enzyme responsible for the degradation of 2-arachidonoylglycerol (2-AG), the most abundant endocannabinoid in the central nervous system. By inhibiting MAGL, RO7268489 increases 2-AG levels, which is hypothesized to exert neuroprotective and anti-inflammatory effects through the activation of cannabinoid receptors (CB1 and CB2). Additionally, blocking MAGL reduces the production of arachidonic acid, thereby decreasing the synthesis of pro-inflammatory prostaglandins. RO7268489 is currently being evaluated in a Phase 2 clinical trial (NCT05862168) as an add-on therapy to ocrelizumab in participants with primary progressive or secondary progressive multiple sclerosis to address neurodegenerative aspects of the disease.
