RO7791094 (also known as RG6122) is an investigational bispecific monoclonal antibody developed by Roche for the treatment of advanced hepatocellular carcinoma (HCC). It is designed as a T-cell bispecific (TCB) antibody that simultaneously targets Glypican-3 (GPC3), a cell-surface oncofetal antigen highly expressed in HCC, and the CD3 epsilon subunit of the T-cell receptor complex. By bridging GPC3-expressing tumor cells and cytotoxic T cells, RO7791094 facilitates the formation of an immunological synapse, leading to T-cell activation and the subsequent directed lysis of the cancer cells. The drug is currently being evaluated in Phase 1 clinical trials as a monotherapy and in combination with other agents, such as the PD-L1 inhibitor atezolizumab, to assess its safety, tolerability, and preliminary anti-tumor activity in patients with GPC3-positive advanced HCC.
