Overview

This is a combination regimen consisting of four agents: **robatumumab** (a fully human monoclonal antibody targeting the insulin-like growth factor-1 receptor, IGF-1R), **vincristine** (a vinca alkaloid that inhibits microtubule formation), **doxorubicin** (an anthracycline topoisomerase II inhibitor), and **cyclophosphamide** (an alkylating agent). - **Robatumumab** binds selectively and with high affinity to IGF-1R on tumor cells, blocking ligand-dependent activation, leading to receptor degradation, reduced intracellular signaling, and antibody-dependent cellular cytotoxicity. - **Vincristine** binds to tubulin, inhibiting the assembly of microtubules necessary for chromosome separation during mitosis, resulting in cell cycle arrest at metaphase. - **Doxorubicin** intercalates into DNA and inhibits topoisomerase II, blocking DNA replication and transcription and inducing DNA strand breaks. - **Cyclophosphamide** is a prodrug that is metabolized to active alkylating metabolites that form DNA crosslinks and promote cell death. This combination potentially targets multiple mechanisms in cancer cell survival, proliferation, and metastasis, and has been investigated in clinical trials for various cancers including sarcomas and breast cancer[1][3][5][6][7].
